Immunization Requirements Missouri State Law Bill Number 167.181
All district pupils will be expected to meet immunization requirements on the FIRST DAY of school. These would include all students unless noted otherwise:
l. Diphtheria (DTP, DT or TD) A minimum of four (4) doses required for grades K-1 and three (3) doses required for grades 2-12. One of the three doses must be after the fourth birthday.
Pertussis is required for all students under 7 years of age. An additional dose of TD is required l0 years after the last dose. Repeat every l0 years throughout life.
2. Polio (TOPV) A minimum of three (3) doses required of Trivalent Oral Polio vaccine, one of the three doses must be after the fourth birthday. (If a combination of IPV/OPV is received, four (4) doses are required.)
3. Rubella (German Measles or 3 Day Measles) One dose required of live rubella vaccine, received twelve (12) months of age or greater.
4. Measles (Rubeola, Hard or Red Measles) Two doses which must be administered at least 28 days apart on or after the first birthday.
5. Mumps (MMR) One dose of mumps immunization on or after the first birthday.
6. Hepatitis B Three doses of Hepatitis B vaccine are required for all students grades K-3 and grades 7-8 during the 2000-2001 school year. (A two-dose option may be used for grades 7-8.)
Missouri School Law requires that children who are not completely immunized will not be allowed to enroll in school and will be excluded immediately. If you have any questions regarding your child's immunization status or for more detailed information, please check with your school nurse.
Preschool Immunization Requirements: The following explains what age-appropriate immunizations are required for preschool attendance to be in compliance with the law.
Required immunizations by the time a child reaches upper age limit are: 0-14 months 3 DTP, 2 OPV
15-17 months 4 DTP, 3 OPV, l MMR
18 months to school entrance 5 DTP, 4 OPV, l MMR, l HB (HbCV)
0-18 months 3 HB

· All students must present documentation of month, day, and year for each immunization before they attend school.
· All immunizations must be up-to-date before students are permitted to attend classes.
· The Advisory Committee on Immunization Practices (ACIP) allows a 4-day grace period, so students in all grade levels may receive immunizations up to 4 days before they are due.
· For children beginning kindergarten during or after the 2003-04 school year, required immunizations should be administered according to the current ACIP Schedule (http://www.cdc.gov/nip/recs/child-schedule.htm#Printable). (See attached)
· To remain in school, students "in progress" must have an Imm.P.14 form on file and must receive immunizations as soon as they become due.
· Religious and medical exemptions are allowed. The appropriate exemption card must be on file (Imm.P.11A or Imm.P.12).

1. Last dose on or after fourth (4th) birthday and last dose of pediatric pertussis before seventh (7th) birthday. Maximum needed: six (6) doses.
2. Last dose on or after fourth (4th) birthday. If a combination of IPV/OPV is received, four (4) doses are required. Maximum needed: four (4) doses.
3. A statement signed by parent/guardian or physician indicating the month and year the child had chickenpox disease.
4. The ACIP and the Missouri Department of Health and Senior Services recommend Tdap, which contains pertussis vaccine, at age 11-12 years for those who have completed the recommended childhood DTP/DTaP vaccination series and have not received a Td booster dose. However, pertussis vaccine is not required for adolescents for school attendance at this time. A Td booster is required ten (10) years after the last dose of DTaP, DTP, DT, or Td.
5. Students who are 11-15 years of age may receive two doses adult formulation Merck Recombivax Hepatitis B vaccine on the following schedule:
